Please be advised that all information is to be TYPED into the fields before submission to the EMSA office.
 
Applications for New Clubs and Applications for Club Change of Status will be accepted annually from November 1 to November 30 for the following year.

Applications will be accepted in 2017 for 2018 from Nov 1 - Nov 30, 2017.

CLUBS WILL BE RECEIVING THE AGM PACKAGE FOR 2016 BY E-MAIL IN DECEMBER
The preferred method of submission of your Club Membership Renewal is by e-mail to the District Administrator emsada@rogers.com
   
Renewal Form for Current EMSA Club Members
This is a fillable form.  Please type the information in the required spaces with an appropriate sized font and then print to have members sign the form before scanning and submitting to the EMSA office via e-mail. Clubs that submit incomplete forms will be denied membership.
EMSA Club Application Checklist
This is a fillable form and all information must be typed except where signatures are required. This must be scanned and submitted with your Membership Renewal Application, a copy of your Club Constitution, Financial Statements and the club's Volunteer Screening and Harassment Policy as per Section 5 Procedure 5.0 Membership in a District Association of the OSA governing Documents.
EMSA is requesting these documents be submitted via e-mail on or before January 8, 2016.

OFFENCE DECLARATION FORM
This form is to be downloaded by the CLUB and the club logo is to be inserted in the space provided.  The form is to be completed and signed by the coach/manager/volunteer and kept with the club files. These files must be available at any time to the district, the OSA or authorities.  The club is to submit a summary of volunteers screened to the district and signed by the club official(s). The club must have a screening policy that indicates the time frame for which a new police check must be done keeping in mind that the police check is only good to the date it was completed.
