PLAYER REGISTRATION FORM - this form must be completed and submitted with all registrations that are not done in the AIMS portal.
COACH REGISTRATION -**NEW FORM** this form must be attached to the printed roster and be submitted with the coach book for ALL teams.

2012 Senior Teams will be regsitered in AIMS by the clubs that currently have training for the youth teams within the club. Those clubs with Senior teams only will still be registered at the EMSA OFFICE.
PLEASE ENSURE YOUR TEAM IS REGISTERD BEFORE APPLYING TO PARTICIPATE IN ANY TOURNAMENTS OR EXHIBITION GAMES!
CLUB'S THAT HAVE HAD THE REGISTRAR TRAINED FOR AIMS YOUTH REGSITRATIONS ARE NOT REQUIRED TO HAVE THE FORMS BELOW FILLED IN AS LONG AS THE CLUB REGISTRATION FORMS MEET ALL OF THE OSA'S MINIMUM REQUIREMENTS FOR PLAYER REGISTRATION.
MR -Mini Recreational - Houseleague Teams L7 (maximum 14 players)
MIO -Mini Intermediate - LDYSL Teams L6 (maximum 14 players)
YR -Youth Recreational - Houseleague Teams L7
YIO -Youth Intermediate - LDYSL Teams L6
YC -Youth Competitive - OYSL, WOYSL, LDYSL L1, L3, L4, L5
PLEASE NOTE THAT THE DATE FORMAT HAS CHANGED BE SURE TO ENTER THE BIRTH DATE OF THE PLAYERS IN THE CORRECT ORDER
